and send the following documents to VisaHQ:
  • Visa application form. Important Note: Please upload or send us electronically the following documents needed for the generation of your application form:

    • Scanned copy of passport
    • Passport photo

    Do not mail anything to VisaHQ without the generated application form.

    Once application form has been generated, please also note that ALL 2 PAGES of your application require your original signature. Each page of your application must be printed SINGLE-SIDED only. Applications printed/signed double-sided will be rejected.

  • Original passport. The passport must be valid at least 6 months after the intended date of departure from the territory of the destination country and has at least 2 blank visa pages.

  • Passport-type photograph. Include a passport style photo, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a charge associated with this service.

  • Complete itinerary. A copy of round trip tickets or detailed itinerary showing all intermediary countries between your destination and return to country of origin. The applicant's name must be on itinerary.

  • Yellow fever . A copy of the International Certificate of Vaccination (ICV) for yellow fever. To learn more about the yellow fever vaccination or to locate a vaccination clinic, please visit the CDC's yellow fever resource page .

  • State Department Letter. A letter from the US Department of State addressed to the "Embassy of Bolivia (Plurinational State of), Washington D.C. ", giving a detailed description of the purpose of travel, the names and addresses of any organizations to be visited, and the dates of the trip. The letter must contain the applicant's name and the passport number and should have a statement confirming that the US Embassy in Bolivia (Plurinational State of) will take full responsibility for the applicant.

  • Proof of status. A copy of immigration status in the country of residence such as a resident card, work, or study permit.

  • Bank statement. A copy of the applicant's most recent monthly bank statement showing proof of sufficient funds. The statement must clearly show the applicant's name as the account holder, the balances of the accounts, and the date of the statement.

  • Hotel Reservation. Hotel reservations for the duration of the trip. The applicant's name must be on the reservation. Reservations must be from the hotel directly or hotel reservation provided by a traveling agency. No reservations from internet booking websites or similar will be accepted.

  • Personal invitation. If visiting friends or family, you must provide letter of invitation with the contact information of the host and visitor, purpose and duration of the visit, confirmation of accommodation including the address, signature and date.

  • Letter of Authorization. A letter authorizing VisaHQ to submit paperwork and pick up the passport from the embassy upon completion of the visa process. Please complete Authorization Letter here.

  • Electronic copies of documents. Upload copies of documents. Upon placing your order please upload electronic scans or photographs of your passport information and your photo. These scans are necessary for VisaHQ to generate your application form.

  • Taxpayer Identification Number. A certified copy of the Tax Identification number or original electronic certification if the country was established in Bolivia.

  • Business registration. A copy of the host organization's Certificate of Incorporation or business registration in the country of destination.

  • Proof of status. A copy of immigration status in the country of residence such as a resident card, work, or study permit.

  • Business Letter. A soft/hard copy of business letter from the sending company. The letter should be printed on the company letterhead stationery, addressed to "The Consulate of Bolivia (Plurinational State of), Visa Section", and signed by a senior manager. The business letter must adhere to the following guidelines: Briefly introduce the applicant (please specify employment status/position held in the company by applicant). State the nature of the business to be conducted (i.e. nature of contracting, etc.) and the names and addresses of companies to be contacted in Bolivia (Plurinational State of). Specify the Type and desired Validity of the visa (i.e. a one year multiple entry business visa). Guarantee of sufficient funds for travel. In the event the applicant is self-employed, please mention the nature of business conducted in Bolivia (Plurinational State of) in the application under "Other" in paragraph 1.15. Specify the place of accommodation in Bolivia (Plurinational State of).

    Bolivia COVID-19 travel restrictions

      Bolivia's air borders are open for travel, and many domestic restrictions are lifted.
      Prior to arrival restrictions
      Valid passport. Valid national passport with at least 6 month remaining validity after the date of departure.

      Negative COVID-19 test. Travelers entering Bolivia are required to present negative PCR test results, no older than 72 hours, and a proof of health insurance covering COVID-19 treatment.
