Original passport. The passport must be valid at least six months after the intended date of departure from the territory of Uganda and has at least two blank visa pages.

Proof of status. A copy of immigration status in the country of residence such as a resident card, work, or study permit.

Itinerary. A copy of round trip tickets or detailed itinerary for the duration of the trip. The applicant's name must be on itinerary.

Personal invitation. If visiting friends or family, you must provide letter of invitation with the contact information of the host and visitor, purpose and duration of the visit, confirmation of accommodation including the address, signature and date.

The business letter must adhere to the following guidelines: Briefly introduce the applicant (please specify employment status/position held in the company by applicant). State the nature of the business to be conducted (i.e. nature of contracting, etc.) and the names and addresses of companies to be contacted in Uganda. Specify the Type and desired Validity of the visa (i.e. a one year multiple entry business visa). Guarantee of sufficient funds for travel. In the event the applicant is self-employed, please mention the nature of business conducted in Uganda in the application under "Other" in paragraph 1.15. Specify the place of accommodation in Uganda.

Business invitation. A business invitation letter from the sponsoring company in the destination country. The letter should be printed on company letterhead stationery, addressed to "The Consulate of Uganda, Visa Section", and signed by a senior manager. The business letter must adhere to the following guidelines:
Briefly introduce the applicant (please specify employment status/position held in the company by applicant).
State the nature of the business to be conducted (i.e. business meetings, contract negotiations, etc.) and the names and addresses of companies to be contacted in the destination country.
Specify the Type and desired Validity of the visa (i.e. a one year multiple entry business visa).

The letter must contain the applicant's name and the passport number and should have a statement confirming that the US Embassy in Uganda will take full responsibility for the applicant.

Passport-type photograph. Include 2 passport style photos, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a charge associated with this service.

State Department Letter. A letter from the US Department of State addressed to the \"Embassy of Uganda, Washington D.C. \", giving a detailed description of the purpose of travel, the names and addresses of any organizations to be visited, and the dates of the trip. The letter must contain the applicant's name and the passport number and should have a statement confirming that the US Embassy in Uganda will take full responsibility for the applicant.

Personal letter. A letter stating the reason for transit and the country of destination.

Daily Travel Plan. A completed daily travel plan is required by the destination country, please include destinations to be visited at specified times.

Itinerary. A reservation of the flight itinerary under the traveler's name. Must show flights entering and exiting Uganda.

Payment Authorization. Complete and sign the Offline Credit Card Authorization Form.

Itinerary. A copy of round trip tickets or detailed itinerary for the duration of the trip. The applicant's name must be on itinerary.

Yellow fever . A copy of the International Certificate of Vaccination (ICV) for yellow fever. To learn more about the yellow fever vaccination or to locate a vaccination clinic, please visit the CDC's yellow fever resource page .

Proof of accommodation. A copy of hotel reservations or other lodgings for the duration of the trip. The applicant's name must be on the reservation.

Personal invitation. If visiting friends or family, you must provide letter of invitation with the contact information of the host and visitor, purpose and duration of the visit, confirmation of accommodation including the address, signature and date.
